You're about to enroll in Avery X-Ray School X-ray Technician class.

You must complete all the steps, below, to enroll in a class:

1. Complete the enrollment form
2. Select a class date
3. Select payment method: one (1) payment of $1,895 or two (2) payments of $947

You may choose to complete the enrollment form in one of three methods.

ONLINE

By MAIL
Print the enrollment form.

Fill out the information and mail the form with your Personal or Company Check, or Money Order before the class starts. Send to our billing office at the address below.

BY FAX
Print the enrollment form.

Fill out the information, and FAX the form to our billing office at (214) 912-3958.

When we receive your enrollment form and payment we will mail or email you a confirmation letter.

Refund Policy: 100% refund available if we receive a written cancellation notice 72 hrs before the first class starts. Refunds will be issued within 21 days.
